Referral Process

Sleep HealthCenters^® has a Centralized Scheduling Office located in our Bedford facility.  The office is staffed Monday through Friday from 8:30 am to 5:30 pm.  In order to schedule an appointment, appropriate documentation is required.  Sleep HealthCenters^® has a referral form that is used to request specific appointments, from consultations with sleep specialists to overnight sleep studies.  In order to request an appointment, the referral form must be fully completed and faxed to our scheduling fax number, (781) 271-0601, along with a copy of the patient’s History & Physical or most recent Consult Note.  Please note: Referrals to the Sleep HealthCenter^® at Beth Israel Medical Center should be faxed to 866-799-0601. A Patient Care Coordinator will contact the patient by phone within 24 to 48 hours to schedule the requested appointment.  A letter will be mailed to the patient if we are unable to contact the patient by phone.
