To speak to your sleep physician, please contact them directly or call Sleep HealthCenters at (855) 390-1136 and we can provide you with their relocation contact information.

Sleep HealthCenters will continue to maintain and protect your medical records. You are not required to transfer them if you do not wish to. However, if you need to request copies of your medical records, or need copies to be sent to other health care providers, you must complete and submit a standard Medical Records Release Form, which can be found below.

If your care has been compromised or there has been a violation of your patient privacy, please call the PCO at (800) 577-5099 or send an email to: ombudsman@winterharborco.com. If you need to order your sleep supplies, request a copy of your sleep study or locate your sleep physician, please call Sleep HealthCenters at (877) 753-3742. The PCO will not be able to satisfy these requests.

If you would like to continue care with your current sleep physician, we have updated information on how to contact them. We will continue to provide updates as we learn them.

If you wish to find a new sleep specialist, contact your primary care physician for a recommendation or contact the American Academy of Sleep Medicine (AASM) to select a qualified sleep specialist. An online directory of AASM accredited member centers is available at www.sleepeducation.com, or you can contact the AASM national office at 630-737-9700 or inquiries@aasmnet.org to get contact information for a local sleep center accredited by the AASM.
